The aim of this service is:
To provide a comprehensive and culturally appropriate clinical dietetic service to meet the needs of patients enrolled with Manaia Health PHO.
The dietitians work with patients referred from General Practice and Maori health providers (Ki a Ora Ngati Wai and Ngati Hine Health Trust). - See brochure for more info
Involvement in health promotion activities and community development initiatives are also important components of the role.
The Following Services are Available:
Diabetes Self Management Groups (client self referral)
This is the gold standard of diabetes education and is strongly encouraged for all clients with diabetes able to participate in a group setting. This course is facilatated by a diabetes nurse specialist and dietitian and gives is broad overview of diabetes and nutrition. Diabetes Conversations (indicate preference on referral form)
Small group interactive session using the diabetes Conversation Map on healthy eating and activity for diabetes. Why healthy eating and keeping active are important, the different food groups, what, how much and when to eat and addressing challenges to healthy eating are discussed. Lasts approximately 2 hours with a small snack provided. Calender
Supermarket Tours (client self referral)
For people wanting information on how to choose hrealthy food for diabetes, heart healthy eating and general healthy eating. Dates for the supermarket tours are regularly circulated. After hours tours can be arranged on an as needs basis for people unable to attend during working hours. Calender Please contact Audrey for bookings (438 1015). Brochure
Drop in Information Session (indicate preference on referral form)
Issues mainly discussed in a group and resources/displays will be available. Most appropriate for people with general questions regarding diabetes and weight loss and people wanting more long term casual interactions with a dietitian. Once the referral is received, clients will be sent dates of upcoming sessions. Calender
1:1 Client Appointments (indicate preference on referral form)
The client and their support person discuss their issue and problem solve ways to change their behaviour. The client often sets goals and returns for review as appropriate for up to 6 months.
