Primary Health Care Nursing Position
The need for this position emerged from the work undertaken as part of the ‘Adolescent Health Services in Schools’ project where it was recognised that students enrolled in the Alternative Education (AE) schools and He Mataariki (teenage mothers) often have multiple and complex needs that need to be addressed in a focused and holistic manner. This position commenced in January 2007, and is employed by Ki a Ora Ngatiwai.
The key purpose of this position is to:
Enhance self responsibility and self care with these students, working in partnership with students, whanau, AE tutors, other health and social services
Provide a co-ordinated, culturally appropriate and accessible primary health care nursing service to these students
Provide a comprehensive initial assessment for all students, and develop / implement individualised wellness plans.
Facilitate a seamless approach to on-going care delivery
