Manaaki Manawa
The objectives of this service are:
- To provide an accessible and acceptable community based phase 2 cardiac rehabilitation service targeted to Maori and focusing on reducing inequalities in the area of cardiovascular disease
- To provide cardio-vascular disease awareness raising targeted to Maori and focused on addressing Maori disparities in cardiovascular disease
- To demonstrate positive working partnerships and effective service co-ordination between general practice and Maori providers and between primary/secondary services
Challenges in recruiting a suitably qualified Registered Nurse to develop and manage the service have delayed commencement of this project within Manaia Health PHO, but it is anticipated this will be addressed by September 2006. Te Tai Tokerau PHO have also supported development of the Manaaki Manawa service in the mid and far North, and the vision is to have a service which is based upon best practice guidelines, and which is both responsive to local needs while also ensuring a co-ordinated and consistent approach is implemented across Northland. The Northland-wide service is to be externally evaluated by Massey University, with some of the funding for evaluation having been granted from the National Heart Foundation.
