CVD Risk Assessment & Management Pilot Prog
This funding is to support a targeted cardiovascular risk assessment and management pilot programme in 5-6 general practice settings and two large industrial sites within Manaia Health PHO boundaries. Expressions of interest were sought from general practices to participate in this pilot programme which will take place over 1-2 years and which will encompass the set-up in practices/industry at staged times; initial risk assessment/management of the identified high risk population groups; and follow-up and re-risk screen at one year for those with an identified CVD risk of 15-20%, or > 20%.
Goal:
To risk assess, manage and reduce absolute cardiovascular risk in identified high risk population groups (Maori/PI; Quintile 5)
Objectives:
To develop and implement a range of approaches to targeted CVD risk assessment and management in the pilot sites
To evaluate effectiveness of the pilot programmes
To ascertain nursing hours required to undertake the programme
To promote the use of best practice guidelines
To encourage general practice teams to develop linkages with communities
