Healthy Hikurangi

Manaia PHO is co-ordinating a two year community action project focusing on heart disease and diabetes funded directly by the Ministry of Health.  The project aims to improve the health and wellbeing of the people of Hikurangi through three key areas:

  1. by working with the whole community to identify, plan and take action to address community level determinants of heart disease and diabetes,
  2. by working with people at risk of heart disease and diabetes, and
  3. by working with people who have heart disease and diabetes.

The project objectives are:

  1. To strengthen relationships between the Hikurangi community, primary health care and social service providers and community groups.
  2. To increase involvement and participation of the Hikurangi community in actions to improve health and wellbeing.
  3. To develop interventions which impact on the determinants of cardiovascular disease in the Hikurangi community.

A community steering group (made up of Hikurangi residents as well as representatives from various Hikurangi community groups) and a health services steering group (involving representatives of the various health services working in Hikurangi) have been established to implement the project.  

The project is based on key principles of inclusiveness, participation, equity and a strength based community building perspective.  Two of the first steps will be to identify people with or at risk of heart disease and diabetes and to carry out a broad community needs analysis.  The project will be extensively evaluated for process and impact/outcomes.
